Dear Patient:

We're pleased that you have chosen Complete & Compassionate OB/GYN Care. Dr. Troy Robbin Hailparn is a board-certified OB/GYN doctor who takes great pride and joy in treating and educating her patients.

On the following pages, you will find "new patient" forms. Please print and fill them out and bring them with you to your next appointment.

We ask that you arrive to your first appointment 30 minutes earlier than your scheduled time. This will help give us time to prepare your chart and have it ready by your scheduled appointment time.

Please allow up to two hours for your first appointment (from the time you arrive to the time you checkout).

When you come for your first visit:

• Bring your insurance card and picture identification (current texas drivers license or other picture id)

• Bring either the medication bottles or a list of medications that you are currently taking.

• Be prepared to pay for any deductibles, co-pays or co-insurance payments required by your insurance carrier at the time of your visit. We do collect all copays before your exam. For your convenience, we accept Mastercard, Visa, Discover and American Express. (It is your responsibility to know and understand your insurance benefits. We try to pre-verify all insurance information.)

Please note that our office hours are Monday-Thursday from 8:30 a.m. to 5 p.m. and Friday from 8:30 a.m. to 3 p.m. The office is closed every weekday from 12 noon to 2 p.m. for lunch. Appointments must be made in advance. We DO NOT accept walk-in appointments. Our goal is to reduce your wait time as much as possible.

Cancellation and No-Show Policy

A $50.00 fee will be charged for each appointment you fail to keep without a 24-hour advance notice. Missing more than three appointments will result in dismissal from the practice.
